Privacy
CarePoint Hearing is a small, local practice. This page explains, in plain words, what this website collects, why, and where it goes.
What we collect
When you call, text or email us, we receive whatever you choose to share, like your name, phone number and a question.
When you fill out the New Patient Questionnaire, we collect:
- Your name, phone number, email address and the best time to reach you
- Whether you are a new or current patient
- What brings you in, in your own words
- Your hearing history: whether you have had a hearing test in the past five years, whether you wear hearing aids, and their brand, age and any issues
- Whether you would like a home visit, whether you are on Medicare Advantage, and how you heard about us
- Anything else you want Melissa to know
Why we collect it
Only to prepare for your visit, contact you about your appointment and care, and understand how people find us. We do not sell, rent or trade your information, and we do not use it for advertising.
Where it goes
- Your questionnaire answers are emailed to Melissa's practice inbox, and a confirmation is emailed to you. We use a service called Resend to deliver those emails.
- When you pick your appointment time, you move to our scheduling calendar at myhearingportal.com, which has its own privacy policy.
- The website is hosted by Cloudflare. Like any website host, it sees basic technical details such as your IP address and browser type.
Cookies and tracking
We use Google Analytics to count visits and see which pages help people find what they need. It sets a small analytics cookie that does not identify you by name. We do not use advertising cookies, and we do not sell or share analytics data. If you block cookies in your browser, the site still works.
How long we keep it
We keep your information as long as we need it to care for you, or as long as the law requires, and no longer.
Your choices
You can ask us what we have, ask us to correct it, or ask us to delete what the law allows us to delete. Call or text (623) 255-8098 or email info@carepointhearing.com.
Children
This website is not meant for children under 13, and we do not knowingly collect their information online.
Changes
If we change this page, we update the date at the top.
